Description

 SUMMARY:

The Social Worker seeks to enhance the quality of life of Eliza Bryant Village housing tenants through planned events and activities. The individuals, government agencies and organizations that can provide programs and benefits designed to meet the needs of seniors to develop and maintain a healthy, secure, and well-balanced lifestyle while living on campus.

DUTIES AND RESPONSIBILITIES:

  • Meet regularly with the Director of Social Services to monitor the effectiveness of the program
  • Prepare a written summary of activities to the director weekly
  • Work with Eliza Bryant Village tenants to ensure communications related to campus activities (social, educational, and health related)
  • Complete individual needs assessments and goal planning with each tenant to determine basic necessities.
  • To facilitate monthly programs such as trips, programs, arts & crafts. Coordinate regular activities that recognize and celebrate tenants, milestones, special and current events, holidays.
  • Participate in Resident Council (attend meetings/events)
  • Perform case management with benefits and entitlements
  • Make referrals for healthcare services such as nutrition, chore service, rehabilitation, adult day care
  • Make arrangements for community-needed transportation
  • Administer survey questionnaires (pre and post) to tenants; maintain the data
  • Other duties as assigned

Education Requirements: Must have LSW

Experience Requirements: Experience working with older adults with a knowledge of available senior services

Other skills and certifications: Excellent interpersonal and project management skills. Computer proficiency. Proficient with MS office applications, specifically Excel. Must have leadership, interpersonal communication skills, problem solving and decision making.
