Salary Range:$54-$58

Responsible for the timely and accurate completion of the MDS (Minimum Data Set) assessments including completion of the CAAs triggers and summaries and individualized resident care plans. Maintains and promotes professional nursing practices and provides clinical and managerial support for the nursing department.


  • Responsible for the completion of MDS assessments (including CAAs and individualized resident care plans). Provides weekly MDS calendars to the nurses on the unit informing them of those residents who need to have MDS documentation for the observation period. Places and gathers all supporting documents (ADL trackers and Mood and Behavior trackers) for applicable residents during assessment period.
  • Communicates with staff (all shifts) to solicit information to ensure an accurate resident assessment. Offers clinical input and communicates assessment findings to Unit Manager and/or staff for care improvements. Assures signatures of team members are obtained. Files MDS assessments in a timely manner once team members have signed assessments.
  • Trained and serves as a back up in the scheduling of MDS assessments based on the state and federal guidelines. Notifies all applicable IDT (Interdisciplinary Team) members of assessment due dates. Participates in care conference meetings as needed. Establishes and maintains collaborative relationships with IDT members to facilitate documentation process. Updates and make changes to the resident care plan in accordance with state and federal guidelines.
  • Responsible for checking that MDS assessments are accurate prior to sealing records. Trained and serves as a back up in the electronic submission process. Performs test submissions prior to sending the final submission to the state database. Trained to make modifications and corrections of errors as needed.
  • Trained to generate state QI reports and provides these reports to appropriate

administrative staff (DON, Administrator) upon request.

  • Understands assessment coding and the impact on the facility's CMI. Strives to identify methods of CMI maximization and helps to identify opportunities for additional MDS  assessments based on the clinical picture of residents and identify significant changes.  Collaborates with the therapy department for maximization of reimbursement.
  • Assists front line staff and participates in emergency care to residents as needed. Maintains current clinical/assessment skills. Functions as a unit nurse PRN for staffing shortage.
  • Miscellaneous: Maintains a record of acceptable attendance and punctuality. Observes all company rules and safety procedures. Performs other duties as assigned by DON or Administrator


Must be a graduate of an accredited school of nursing with a clear and active license as a Registered Nurse/Licensed Practical Nurse in the state of Ohio.


3-5 years experience with PDPM, PPS, MDS 3.0 experience required. Must have a current working knowledge of the state and federal guidelines for completing MDS assessments, CAAs and care planning process. Must maintain a current license and practice nursing in accordance with the Nurse Practice Act.

Physical demands may include kneeling, climbing ladders, reaching, pushing, manipulating, talking, hearing, standing for extended periods, crouching, stooping, walking, lifting up to 75 pounds, transferring up to 150 pounds and repetitive motion. Must possess the dexterity to prepare medications and apply treatments.  MDS Nurse must be able to use the telephone, computer and communicate effectively.
